Shoulder Instability and Joint Position Sense

This is a mini-blog with the intent of giving the user of the Motion Guidance visual feedback product insight into a researched-based approach to using the product for the diagnosis of shoulder instability. Movement joint positionsense is most likely to be impaired after shoulder injury involving post-traumatic instability when compared to the contralateral shoulder and to controls, while deficits for Active JPS and Passive JPS are more likely to be evident compared to the contralateral shoulder in participants with glenohumeral musculoskeletal disorders. This has been shown in a 2011 study by Balke. A link can be found here . Balke was able to demonstrate that angle reproduction capability was worst below shoulder level (55°) in all groups. Best results were achieved at shoulder level (90°). Healthy controls showed overall better results than patients with instability. Research Shows Massage Impacts Recovery, Training and Flexibility of Elite Para-athletes Research on the value of massage therapy for elite para-cyclists has been published for open access online by the British Medical Journal: Sport & Exercise Medicine . Funded by the American Massage Therapy Association (AMTA) through Team Roger C. Peace, the research followed the impact of massage therapy on para-cyclists as they trained and competed in national and international events including the 2016 Summer Paralympic games in Rio. Ankle Sprain Motor Control and Mobility

After a bilateral ankle inversion sprain, we had to progress slowly with motor control to enhance single limb stance stability. Prior to single limb stance, I wanted to make sure the patient had good motor control in a lunge.  The first exercise is a Triplanar Lunge. First she lunges forward, trying to keep the laser on the straight forward line. After a few minutes, she is instructed to keep the foot forward as much as possible, lunge on the lateral line. This should emphasize lateral tibial glide and tibial IR, while gently challenging her ankle ligaments. The next instruction is to again keep the foot forward, then try to keep the laser on the medial facing line. This should emphasize medial tibial glide/ER while also lightly challenging the sprained lateral ankle ligaments. Lastly, holding the lunge, she is instructed to move the laser horizontally along the perpendicular line left to right. This should work again on tibial IR/ER and lateral/medial tibial glide. I spent many hours in 2017 contemplating the importance of one key element of human survival - breathing. A Novel Mobilization for Acute Ankle Inversion Sprains

The case in the video was a severe ankle inversion sprain on the left and moderate on the right. The patient is a high level gymnast who injured herself tumbling during a floor routine. The video below was taken almost a week after the sprain. She had significantly decreased mobility with pain in all planes. Normally I would try a posterior glide to the lateral malleolus and ankle inversion/plantarflexion, which has worked on her in the past with great results. This time, that was too painful as was lateral malleolus contact.  Instead, I tried slacking the irritated skin and ligaments while gradually moving the forefoot/ankle into dorsiflexion and eversion. This was pain free and after working my way to end range, her active/passive NWB range was much better and her gait was nearly pain free. This year, I am completing my first  decade   of work as a Physical Therapist—wow, can’t believe it.  I believe I am now considered an oldie…the dinosaur…in our field! edit: Told the author (Dr. Harrison Vaughn) - that I have been out almost 20 years - what came before the dinosaurs 😜 - Dr. E - now back to the post I hope all of my colleagues and readers are continuing to enjoy the profession. Like you all, I continue to have the challenges, failures, and tribulations with clinical practice.  Hopefully these get fewer and fewer between but still  very  normal even with experience—its the beauty of physiotherapy…of healthcare.   I continue to aim high and hopefully the defeat gets buried away, hidden underneath the hours you spend beating on your craft.  Continue to pursue greatness to propel yourself and our field.  Always remember, have fun as we are very  fortunate  to  change  lives everyday.
