Functional Mobilization for Ankle Pain in Deep Squat

Image
The lateral tibial glide is a great mobilization that restores ankle dorsiflexion. It often improves "knee stability" as well because it enables the tibia to glide efficiently over the talus instead of moving medially when restricted in lateral glide. This is a progression in full deep squat - the patient is a pediatric PT as well as a catcher and needs to have full mobility when shifting around in a squat position. Remember, to even get to pain free full squat, she needed to restore sidegliding in standing to the right, or ability of the lumbar spine/hip to accept load. Only after that mobilization was she able to progress to the lateral tibial glide in deep squat. Top 5 Fridays! Critical Eval Questions You Should Be Asking

Image
We've all been guilty of "cutting to the chase." Or in my traditional OMPT days, cutting off the patient's long and complicated history so you can get right to passive care! I've incorporated many of these questions into my evaluation, and the initial patient encounter is sometimes 3/4 of my evaluation. It's my favorite part of the visit. Here's a great quick slideshow of Critical Questions (more than 5 so bonus for this Friday) you NEED to be asking during the evaluation. Top 5 Fridays! 5 Things You Don't Learn About Pain Science

Image
Pain Science Education is one of the best things to ever happen to the rehab profession. I remember just how eye opening and mind blowing David Butler's first Explain Pain course was in 2002. Yes, I took it 17 years ago. I remember thinking, this is going to change EVERYTHING! 5 Things You Don't Learn About Pain Science  use the arrows to advance the slides Text placeholder by Erson Religioso III It did and it didn't. I've been using many of the metaphors, stories, research snippets, and other ways in my approach to patient education and interaction since then. My experience is that those who need it the most, are those who respond the least or are the most skeptical, close minded, or downright upset over your so called "explanations." I still recommend taking some version of pain science course, if only to learn how to dispell myths and make patient less dependent on passive quick fixes. You'll do all of us a favor who are trying to get pat...

A KISS for Valentine's Day: Keeping It Simple...Sort of

Image
Everything should be made as simple as possible, but not simpler -- Albert Einstein We've all heard of KISS, No, not the rock band that grew to fame in the 80's. I'm talking about the phrase  "Keep It Simple, S....(insert your word of choice)" Within our profession, there has been a propensity to make things more complex than necessary Why is that? There are several possible explanations. I heard Greg Lehman (@GregLehman) make an interesting observation while on the Chews Health Podcast (@TPMpodcast) and more recently the NAF Podcast with Adam Meakins (@AdamMeakins): "There are a lot of smart people in the profession. It's hard to get in [to school] and you work really hard and I think there's a desire to do something that's complex because simple makes it seem that anybody can do it."  That does make a lot of sense. However, do patients want what's simple? Ideally, I think so, but I so know that if we each reflect upon our own...

HHS Issues Call for Comments on Pain Task Force Report - AMTA Comments

Image
As AMTA announced in early January, the U.S. Department of Health and Human Services (HHS), through an Inter-Agency Task Force, has published a draft report on pain management best practices. This draft calls for individualized, patient-centered pain management, and it includes recommendations related to massage therapy. Your Opportunity to Comment AMTA has provided its comments to the task force and encourages those massage therapists engaged in healthcare and those with interest in providing massage therapy in collaboration with healthcare facilities and insurance to comment on the massage therapy portions of the draft guidelines. All comments must be received by their deadline of April 1, 2019 . AMTA continues to be actively engaged with HHS and the Task Force to work toward massage therapy’s full inclusion as part of the recommendations. This has included direct meetings with the chair of the Task Force and other stakeholder groups. AMTA has commended the Task Force for it...

CMS Encourages Medicare Advantage Plans to Cover Massage in 2020

AMTA continues to engage the Centers for Medicare and Medicaid Services (CMS) about full inclusion of massage therapy under Medicare Advantage programs as an option for members in licensed states who wish to be involved. In January, CMS wrote to all Medicare Advantage programs, taking it one step further to include massage therapy as a covered benefit. In their recommendations for 2020, they encourage the plans to cover massage therapy, rather than just saying it would be accepted by CMS as they indicated for 2019. The CMS letter now says the following about “ Non-Opioid Pain Management Supplemental Benefits” “CMS encourages MA (Medicare Advantage) organizations to consider Part C benefit designs for supplemental benefits that address medically-approved non-opioid pain management and complementary and integrative treatments. For example, “peer support services” delivered by qualified individuals may be effective in facilitating recovery and assist in navigating health care resources....

Top 5 Fridays! 5 Myths in Rehab Social Media and Seminars

Image
🛑Here are a list of myths that are commonly believed and still taught despite there being a wealth of evidence to the contrary. I've believed most of these things at some point in my 20 year career and the reason is always the same. We are victims of our own success. You apply technique or exercise "x" - it works, and the mechanisms or reasons behind why it works or why you're supposed to use it are automagically validated. 🧙‍♂️ I'm not saying don't use these things if they work for you. What I am saying is you can do things differently than the way they're traditionally taught. Don't try to force someone into explanations they're not ready for. Don't use painful treatments, even if research shows it's ok to load a tendinopathy into pain; you're kind of a jerk if you can actually make it more comfortable for the patient using some modulatory technique. What are some of the things that you used to do or believe that you'...
