Rhode Island Passes New Massage Therapy Bill

AMTA is excited to announce that the Rhode Island Governor recently signed a bill that improves standards for the massage therapy profession. Highlights from the bill include:  An increase in initial required massage therapy education from 500 to 650 hours of supervised in-class, hands-on courses and clinical work.  A mandate that the department of health investigate all complaints, including allegations by any person, entity, license or organization related to the practice of massage therapy.  Changes to the examining board to ensure that the majority of its members will be licensed massage therapists.  The new bill which sets the standards for continuing education also requires the board to create an annual report with detailed actions and recommendations. Is Preoperative Education Beneficial? For part 1 of this post click here Does PE achieve any of its goals, and is PE worth the time and financial resources required of it? A 2014 systematic review by Jordan et al. concludes, “There is currently insufficient evidence to recommend routine implementation of pre-operative education programmes (2).” Then there is another 2014 systematic review by McDonald et al. that states, “...we are unsure if it (preoperative education) offers benefits over usual care in terms of reducing anxiety, or in surgical outcomes, such as pain, function, and adverse events” (1). There arises different difficulties when assessing the effectiveness of PE. One difficulty is the heterogeneity as described before. Can Vertigo and Dizziness come from Neck Muscles?

When a clinician thinks of dizziness from the neck, or Cervicogenic Dizziness or Cervicogenic Vertigo,  typically  the zygapophyseal joints come to mind as a proprioceptive and nocioceptive abnormal afferent input. In fact, most authors agree that the following order,  C1-2, C2-3, C0-1 and C3-4 , are the most often influenced in cervical symptoms following mTBI due to high influence of proprioceptive activity from these levels. Moreover, the muscles of the posterior cervical spine, the suboccipital musculature, have an abundance of muscle spindles and are high in mechanoreceptor concentrations.  These deep, short intervertebral neck muscles are also  typically  involved in proprioceptive and nocioceptive abnormal afferent input. Part 1 of this Post Op Progression Series is here - 5 Considerations for Post-Op Patients Over my years of practicing, I’ve realized that many clinicians have no criteria for progressing/regressing their post-surgical patient's rehab programs.  They seem to do it randomly or solely based on time or “just because.” And if you’re anything like me, those reasons (especially the “just because”) doesn’t fly. So here are the thoughts, criteria, etc. that go through my head when deciding on when/how to progress (or regress) patients rehab programs.  I’m not saying these criteria are the only way to do things, the only factors to consider or that they are set in stone, rather these are guidelines I use/consider to safely progress my treatments.  Anything has to be better than nothing.
