The Problem with Traditional Physical Therapy Conferences

Image
As physical therapists, it’s our job to rise above the noise and become influencers in our communities.   I thought that by going to typical physical therapy conferences, I could get closer to that goal. I did what everyone told me: marketed to physicians, took out print ads, sponsored the local football stadium, etc. But it didn’t work.     Patients pay their physician a copay, so why would they pay me (the “technician”) more?  When I was trying to rely on the marketing strategies above, that’s the patient mindset I ran into a lot.  Have you ever had a patient who was prescribed PT three days a week for 12 weeks, but you determined you only needed to see them once a week to get the desired result? What that tells you is this: because you’re just the “technician,” you don’t get to be the decision-maker. If we rely on marketing to physicians, we become technicians instead of the Doctors we are.  Overall, I never found a PT conference where I...

Top 5 Fridays! Develop Power with These 5 Band Resisted Jump Exercises

Image
💥 Band Resisted Jumps 🧨 ⠀ 🎇 Building force production capabilities and the ability to express power are true staples of athleticism. Jumping, whether in the squat position or split squat position, is a key movement skill necessary to excel in athletics. ⠀ 🧠Learning how to jump and land is one thing. However, learning how to jump continuously in short bouts allows you to build a rhythm for better sequencing and coordination. Athletes need to be reactive to do well in sports, so this is a way to add a base layer to that skill. ⠀ 🎯After the base layer has been developed, we can then appropriately dose in reactive jumps to aid in the ability of the athlete to react and produce force. Implementing reactive jumps allows for you as the coach to see how smoothly your athletes can produce a single bout of force at game speed. ⠀ 🔑The addition of the band is another way to overload the system and challenge the athlete with resistance against their jumps. Start with bodyweigh...

Reducing Arthritis Pain Through Exercise

Image
Despite high quality evidence supporting exercise as a first line treatment for the management of osteoarthritis (OA) symptoms, typical management continues to prioritize short term strategies such as injections, medications, and/or surgery. Exercise and OA by Cameron Faller With OA resulting in a significant burden to both individuals and our society, long term solutions are pertinent. This new article dives into the concepts of why exercise can be beneficial for the management of OA which should shift our mindset towards conservative management first.  OA has been considered a whole person disease as two-thirds of people with OA also have chronic conditions resulting in low-grade systemic inflammation. Locally, OA can increase peripheral sensitivity decreasing muscle strength, neuromuscular control, and loading tolerance in the joint. Systemically, OA results in increased inflammatory biomarkers leading to higher states of pain and disability.  Exercise h...

This Is Why You Need to Read Studies and Not Just Look at Abstracts

Image
The following is the conclusion of a 2019  prospective, multi-center, randomized controlled clinical trial  examining the difference between cervical mobilizations and traditional massage in 360 adult patients with the diagnosis of cervical vertigo. “No differences in outcomes were detected between the SCM {Shi-Style Cervical Mobilizations) and TM {traditional massage} groups in terms of treatment of cervicogenic dizziness. Efficacy trials are required to determine whether the improvement observed for each treatment was causally related to the interventions.” From a birds-eye viewpoint, you can take this conclusion that massage therapy is just as good as cervical mobilizations for the treatment of cervicogenic dizziness.  Some may even take this piece of information further to state the  massage therapists  could be the ones  treating  cervicogenic dizziness. Now, I don’t have a problem with massage therapists treating conditions! Heck, we hav...

Top 5 Fridays! 5 Facts About Dynamic Knee Valgus

Image
5 FACTS ABOUT DYNAMIC KNEE VALGUS Dynamic knee valgus has been a topic of debate for many clinicians that work with athletes or others with knee related pathologies. Evidence can be conflicting when relating dynamic knee valgus to injury or pain. To date, there are at least 5 theories that we have gained through research that discuss dynamic knee valgus. 5 Facts About Dynamic Knee Valgus by Cameron Faller 👉Dynamic knee valgus is a normal movement pattern - We see this very often in sports. What we know is that performance of dynamic knee valgus once or a few times is not detrimental, but the more repetitive it becomes, or placed under higher loads, the less it should occur. 👉There is no relationship when screening for knee valgus and predicting injury - A prospective study found no correlation when determining injury risk to the ankle or knee. However, the sample size was small with a low occurrence of injury over the years which may underestimate the results....

Ankle Syndesmosis Injuries – Management

Image
In 2017, Alicia published two great blogs on the anatomy of the ankle syndesmosis and assessment of injuries to this body region. Recently I listened to two podcasts by Clinical Edge with guest speaker Chris Morgan (on assessment and management ), which has motivated me to continue our final blog on the management strategies for ankle syndesmosis injuries. The first podcast covers assessment of injury severity. Unlike other ankle injuries, swelling and pain are not as reliable in indicating the severity of injury. As Alicia discussed in the first blog, injury is often traumatic forcing the joint into hyper-dorsiflexion and external rotation. Morgan et al (2014, p. 602) report that “a large proportion of match related ankle injuries (40%) are due to foul play”. Morgan expands further on this topic during the podcast, reinforcing the importance of understanding the mechanism of injury (MOI) to assist with early diagnosis and classification of injury severity, with the goal of ap...
