Reposted with permission from @rehabscience - ๐Ÿง  The scapulae are flat bones that serve as attachment sites for 18 muscles. These muscles act to move the scapula through six movements, which include: elevation, depression, upward rotation, downward rotation, protraction, and retraction. - ๐Ÿ”Ž As was discussed yesterday, scapular winging involves a lifting of the medial scapular border off of the rib cage and is not a sign of pathology in many cases. In situations where a long thoracic nerve injury has been sustained or shoulder complex hypermobility is leading to functional issues, exercises, like those shown here, can be helpful in terms of regaining scapular control. . 1️⃣Cable Row 2️⃣Scapular Y 3️⃣Landmine Press 4️⃣Shoulder Clocks - The key in this exercise is to prevent the should...

What are Symptoms of Cervicogenic Dizziness?

Image
What are symptoms of Cervicogenic Dizziness? The symptoms of Cervicogenic Dizziness can vary.  This is due to the vague definition of Cervicogenic Dizziness and the confusing aspect that Cervicogenic Dizziness is still called Cervical Vertigo.  This leads us to the symptoms… Cervicogenic Dizziness is not described as true vertigo.  If it is, this is very rare.  Vertigo is defined more as the room or the person is spinning.  That would lead someone to think more of a diagnosis such as BPPV (with the V meaning for Vertigo). Therefore, Cervicogenic Dizziness would not be described as room or person spinning, or true vertigo.  The following are other descriptors and symptoms that are typical from a patient with this condition: Dizziness Lightheadiness Unsteadiness Drunkenness Other descriptors could be: I feel like I may fall People tell me that my head is not on straight I can’t walk straight Additionally, patients may also have symptoms of: ...

Untold Physio Stories Podcast - An Unlikely Dorsiflexion Fix

Image
In this episode of Untold Physio Stories, Erson goes over a recent case working with a runner with chronic knee pain. Part of his limitations were severe hip, lumbar and ankle mobility. Hip and Lumbar were reset easily, but ankle dorsiflexion stretches, resets, and eccentric loading were tried for months until a very rapid fix was accidentally discovered. And by the patient, not the clinician! Top 5 Fridays! 5 Hip Flexor Strengthening Exercises - Tight Hips or Weak Hips?

Image
๐ŸŽฏStrong Hip Flexors - 5 Exercises⤵️ ⠀ ๐Ÿ”‘The often-neglected, commonly complained about "tight hip flexors" can really be avoided if we actually load and strengthen this region of the body. ⠀ ๐Ÿ”‘Most folks fail to load the hip flexor muscles, which are important contributors in all sprinting, running and sporting endeavors that require flexion of the hip to occur. ⠀ ๐Ÿ”‘The key to strengthening this area is to spend quality time under tension via isometrics, eccentrics and through tempo work. ⠀ ๐Ÿ”‘The mini-band is a great tool to begin providing strength work to the hip flexors, which can then be progressed to the utilization of sliders, bands and equipment of that nature. ⠀ ✅Exercise list: ⠀ 1. Screening for VBI and CAD in Patients with Cervical Pain

Image
INTRODUCTION In 2018, the Australian Physiotherapy Association published revised clinical guidelines for safe practice of manual therapy on the cervical spine. This topic of “safety” and in particular, identifying patients presenting with signs of VBI and CAD (vertebrobasilar insufficiency and cervical arterial dissection) is a topic I’ve come back to many times throughout my career. Each time I read the literature updates, the same message is made very clearly. Although both VBI and CAD are fairly rare conditions, their primary symptoms are common reasons that people seek treatment from physical therapists, namely neck pain, headaches and dizziness. The impact of these conditions is serious and potentially life threatening. We rely heavily on our subjective examination to guide us to make relevant adjustments to our physical examination and management. VBI and CAD are contraindications to manual therapy, specifically end range mobilization and manipulation. This is...

Modern SIJ Assessment and Treatment Part 2

Image
Sacroiliac Joint Assessment tests that involve palpation of bony landmarks, and palpation of motion are considered some of the least reliable orthopaedic tests. It was identified by Laslett et al as far back as 2005 that we should use provocation testing and a lumbar repeated motions to rule out lumbar and rule in SIJ. For part 1 of this video series, click here! perform the expected test get a functional test-retest baseline (squat, lunge etc) instead of doing the expected passive treatment, like a manip, MET, mobilization, use a repeated loading strategy like repeated sidegliding or extension retest the function AND the expected test, like supine to long sit - then ask the question, if this is now even, without "putting something back into place, what just happened?"  Modern Manual Therapy is all about avoidance of palpation based motion testing or provocation testing. Why not get right to relief? Do you really need to diagnose by provocation or is alleviation ...
