Why is Recent Infection a Precaution to Orthopaedic Manual Therapy?

In the field of Orthopaedic Manual Therapy, one of the first things we learn are contraindications (relative and absolute) and precautions prior to performing a procedure.  A full list of cautionary measures is beyond this blog post, but one I want to bring to attention is recent infection (particularly throat injection). In conventional clinical reasoning, a recent infection is associated with a risk factor (recent event) for cervical instability but substantially more dangerous of a condition, with fatal consequences, is the arrival of a spontaneous cervical artery dissection.  Frankly speaking, this could be the start of a stroke in process and is an absolute contraindication to physical therapy (or even chiropractic, massage, osteopathic, etc) treatment; but requires medical intervention. Evaluate, Stabilize, and Reset the Hip Part 2

Here is my quick self evaluation, treatment, and strengthening/stabilization of the hip for anterior (front) hip pinching or pain during squatting.  If you missed part 1 - click here ! This is based on the Eval, Reset, and Stabilize System I teach in The Eclectic Approach to Modern Manual Therapy. After your manual therapy, or making loading of the hip threat free, make sure the patient (or yourself) can replicate the effects of treatment repeatedly at home or the gym. The dosage is key to make the improvements last. In Part 2 of this video series, I go over the Stabilization Phase of Eval, Reset, and Stabilize. Senate Committee Testimony Calls for More Coverage of Massage

The U.S. Senate Health, Employment, Labor & Pensions Committee (HELP) recently heard testimony from Dr. Halena Gazelka, Director of Inpatient Pain Services at the Mayo Clinic, on the importance of massage therapy and why insurance should cover it, as an alternative to opioids for pain. AMTA’s representative was at the hearing as the association continues conversations with legislators and federal agencies about massage therapy for pain. AMTA also provided the committee with a written statement on the efficacy of massage for various types of pain and pushed for full inclusion of massage in all guidelines to help overcome the opioid crisis. Cindy Steinberg, National Director of Policy and Advocacy for the U.S. Pain Foundation, also spoke to the importance of massage therapy as an effective part of integrative pain treatment, during her testimony on the opioids crisis. Evaluate, Reset, and Stabilize the Hip Part 1

Functional Mobilization for Ankle Pain in Deep Squat

The lateral tibial glide is a great mobilization that restores ankle dorsiflexion. It often improves "knee stability" as well because it enables the tibia to glide efficiently over the talus instead of moving medially when restricted in lateral glide. This is a progression in full deep squat - the patient is a pediatric PT as well as a catcher and needs to have full mobility when shifting around in a squat position. Remember, to even get to pain free full squat, she needed to restore sidegliding in standing to the right, or ability of the lumbar spine/hip to accept load. Only after that mobilization was she able to progress to the lateral tibial glide in deep squat.
